The patient relationship management (PRM) system is the technology that tracks all communication between the Provider and the patient as well as all extenders that work on behalf of the Provider. This communication includes all modalities, text, phone, email, etc. This tracking is key to understanding when and how the patient is engaging in their healthcare as well as what they need. This system includes a full claims and clinical history on the patient and develops their network of service providers based on this history. It also tracks patient preferences and their special needs. When conducting any medical cost and quality outreach programs, this system allows us to understand what worked and didn’t work by patient. This data allows for on-going tweaking of engagement methodologies until ultimate success.
